TLDR : Answer Summary
To apply for a COVID extension visa at the Chiang Rai immigration office, you need to submit several documents including photocopies of your passport's bio data page and all stamped pages, a photocopy of the TM6 form, a 4x6cm photo with a white background, and the TM7 and TM30 forms. It's also required to provide proof of residence and have an ID photocopy. Ensure you apply at the office corresponding to your registered TM30 address.
◦ Get TM.30 ( back around the corner of the building)
◦ Get photocopy of T.M.30, all visas in passport and T.M.6. All signed by you (photocopy shop is right next to where you get T.M.30)
Order of forms-
◦ Application of extension for temporary stay (on the side where your passport size photo goes white your phone number under where is asks for your signature)
◦ Photocopy of your passport page with T.M.30 stapled above with signature
◦ Photocopy of visas (current and past) with signature
◦ Photocopy of T.M.6
◦ Acknowledgment of terms and conditions of temporary stay in Thailand
◦ Acknowledgment of penalty of visa overstay
◦ Affidavit for granting Permission of temporary stay in the Kingdom during COVID-19 (reason for staging is travel/tourism)
Also you have a 14 day “consideration” period and once those 14 days are up you come back to immigration with your passport and get a stamp for 45 days.
at immigration! It’s the white building in the very very back where they do photocopies. Make sure your landlord or hotel has already put you in the system!
Photocopies of bio data page on passport and all pages with stamps, photocopy of TM6 form, 4x6cm photo w/ white background & neutral expression, TM7 and TM30. The big thing is that you need to show proof of residence and that that person has to provide photocopy of ID, fill in TM30, and show proof of residence registration
