What documents do I need for a COVID extension at Phuket Town immigration?

Hi. I am about to apply for covid extension 60 days at Phuket town immigration

I believe the documents are same as for initial 30 days extension plus an affidavit

However someone says I need a tm 30 ( record of residence) is that right? If so would a letter from my hotel be enough?

 Just write "covid-19" or "Visa run not possible." I wrote "covid" in it and nobody was interested. they want tourists to stay here and spend money... they know what it's all about
