At all the offices except bangkok you don't need anything but your passport (maybe the blue/green receipt paper they gave you when you applied/paid the 1900baht) when you show up on the date stamped in your passport when the under consideration stamp runs out.
In BANGKOK you need to update your bank book make a copy of that page and go to Section L-2.
