I went to Nonthaburi to get the 60 day Covid extension today and they finally hit me with the 2K fine for not turning in a 90 day report. Also the ATM there was broken because it got flooded, so if you go down there make sure you have cash on you.

Nothing special. Passport, departure card, copies of my passport and departure cards, a tiny photo, the TM 30, and the packet of forms they give you to fill out.
