This is the requirement list for non imm O and OA extensions on Koh Samui, and now, also Phangan.

The requirement for a house number photo (Item 12.) is new to me, but no problem.

Item 13. I dont recollect seeing before and dont recognise the numeric 654/2564

Anyone know?

is the number of the police order which was effective on October 2022. That is about changing insurance requirements for those who are applying for an extension based on retirement continuing from Non-OA entry. The changed requirement is bottom of the sheet.

there are a couple 'renegade' immigration offices (the southern Island ones that require a medical certificate to get a year extension on an O, or OA visa based on retirement.
