What are the current document requirements for a 2+1 month tourist visa in Penang, including for a child?

Hi Dears, any latest change in number of docs for 2+1 month tourist visa in Penang? I ve heard now they ask for ticket back obligatory.. So then:

- filled in form,

- copies passport main page and last entry to Thai stamp page,

- photos

- ticket back

- visa fee

- Anything else?

Anything needed additionally for a child visa (teenager) except birth sertificate (English translation) ?
