What are the correct dates to enter in the online application for a 90-day extension in Thailand?

Afternoon all

90 day online application. This is my 4th renewal, previously all done at immigration. My online application has been rejected, I’m assuming it’s due to incorrect dates? What should be the 2 dates I enter into the online application?

Andre, you also need to be aware that if you have less than 7 days left until your 90 day report deadline , then you have to submit in person at your local IO.
