Make sure you have an IDP, as the insurance will ask for this if you claim. Roojai are easy to deal with.

Yes i would definitely recommend first class insurance on a vehicle you will be driving in Thailand whether it is a rental or your own. Contact Rojai or Check DI. Make sure you have appropriate IDP. And discuss with rental company.
