Marriage visa - can I do that while I'm still in Thailand or do I need to get this outside of the country.

You have the STV visa and it cannot be changed into any other visa inside Thailand.
You agreed to accept that rule when you accepted the STV visa.
2.5 STV cannot be changed to any other type of visa during the duration of stay in Thailand.

You still can get married to Thai while on the STV...
Then leave and return to Thailand with a new non O visa as married to Thai and apply for the one year extension of stay.
