All single entry re-entry permits are 1,000 THB, and all multiple entry re-entry permits are 3,800 THB. BKK charges an additional 200THB if they fill out the paperwork.

when you go thru security screening (which you have to do to get to it) tell them you need a re-entry permit. They will track someone down for you if it's not got someone manning the counter

Yes it is. It's past immigration control, so let the immigration officers know you want a re-entry permit before leaving
