At most offices you can apply for an extension when you have 30 days or less left. At Bangkok, Chiang Mai and Phuket you can apply when you have 45 days or less left.

I'm not sure how far in advance you can extend your work permit though since you'll need that for your application as well.

As long as you have all the paperwork you'll get your extension the day you apply.
