TLDR : Answer Summary
If your child is 11 years old, whether they need to apply for their own visa in addition to your multiple entry visa depends on their nationality. If their passport requires a visa to enter Thailand, then yes, they will need to obtain their own visa, independent of your visa status.

Depends on your child’s passport. If their nationality requires a visa then they need their own to enter Thailand. It’s irrelevant what visa you may hold.

How did you prove yours? If you’re in your home country a copy of the passport may well be enough.

with a driver's license. Her passport only shows her state of birth. not our current address

Copy of her passport, yours and your driving licence then. Combine in to one file and upload.
