Do I need two separate documents for a fit to fly certificate and RT-PCR test to enter Thailand?

I am trying to book my sons covid test. In the information he has he needs

1. fit to Fly certificate issued 72 hours prior to departure

2. medical certificate with laboratory result indicating that Covid 19 is not detected and Covid test must be RT-PCR

Does this have to be 2 documents or can the information be on one document from an accredited testing facility. I am finding most places only issue 1 certificate/document with both lots of information. Has anyone entered Thailand with one document covering both points. Please advice.

2 documents. Basically A lab test result for the PCr test and a Dr certificate that you don't have covid (as per PCR result) and that you are fit to fly.

Emirates wanted both when I flew, but Thai immigration only wanted the PCT test certificate

John Monk I was able to get it done in NC but to simplify the process download the app QuickMD they can do your Fit to Fly also look for location in your area that specializes in travel vaccinations in my case it was Arcpoints.. I recieved my passport back 48hrs before I flew so on the date it arrived I had to book my ticket to Cali and foolishly I thought the location near me was processing with a fast turn around no 2-3 days and it was not the correct test after 45 min of calling around the immediate area I googled travel vaccination clinic or labs And easily booked an appoint at a location 45min away without even calling drive there did the test an hour later received my results I schedule my fit to fly on QuickMD for 75$ the day prior

if she was in the US I can make recommendations being that I was able to get my RT-PCR TEST AND FIT TO FLY IN THE SAME DAY WITH 24hrs left until my flight in to Thailand day 2 of quarantine
