TLDR : Answer Summary
The Fit to Fly certificate and the COVID test are distinct documents but often confused. The Fit to Fly certificate indicates that a traveler is fit to board a flight, while the COVID test confirms the traveler is negative for COVID-19. Some services may offer both at a bundled price. It's also mentioned that the Thai government requires them to be treated separately, which adds to the confusion. Travelers may acquire the Fit to Fly certificate through various means, including remote consultations.

It’s not being thick it’s the Thai government confuses it by saying they want it separate most others say the test is a fit to fly covid test

In fact gogodoc just called as my Thai partner will fly tomorrow. The questions were : have you got a cough? Have you lost sense of smell/taste? Have you got a fever? Have you been in contact during the last 14 days with anyone known to have covid? Have you any other symptoms you want to tell me about?

Then the doctor said "If anything changes before you fly, you must report it"

Job done. We paid £22.30 for the appointment (discounted from £30) and £50 for the certificate, which should be emailed within the next couple of hours.

That sounds good and easy, but I doubt you can do that in many countries. Lucky you!
