Do I need to fill out a special form when providing a letter and passport copy for my visa run to Bali?

I'm doing a visa run to Bali. A friend of mine is providing a letter + passport copy to show I'm staying with him in Thailand. Is there a special form to fill in for this? Thank you!

The Bali consulate is closed till the 7th of January including. Went there myself 2 days ago.

Someone told me it is not possible get 1 year multientry b visa from there? I'm going there next month and plan was remake my 1 year b visa again. So has anyone done that before in Bali? Cheers

Not anymore, you will have to apply it in your country, not in a 3rd country. At least, that the rules.

I went there once to get a tourist visa. I was asked to provide flat OWNER's ID copy in addition to rental agreement. All that is a troublesome so I booked a hostel on booking.com and cancelled later. FYI, they asked every single thing listed as requirements.

A visa is a sticker. I can't see them putting it on a passport copy.

It has to be in the passport.
