Note : these forms you can download from immigration website or when you go to visa extension can get from immigration office.

PASSPORT FIRST PAGE COPY.

CURRENT VISA COPY.

TWO PICTURES PASSPORT SIZE.

Proof of address or house hold registration where you are living , one copy.

Owner of house I'd card copy.

1900 THB fee.

I get this extension from GOVERNMENT COMPLEX BUILDING B CHANGWATANA.

no cannot, he is on 60day TV so first extension is the normal 30 days

That is not how it should be. If you really entered on a 60 day Tourist visa (sticker in passport) you first are eligible for 30 days. Maybe because Pakistan passport is different agreement, I know Indian is (sorry I know not same country) or immigration made mistake.

I entered on a TR 60 days tourist visa, did the 30 days extension and then the 60 days Covid extension. Am i eligible for a 2nd Covid extension or 2nd 30 days extension or is that it and will have to go back home ?

you can apply for another 60 days as long as the government allows. The application deadline atm is May 29. You be stamped back to back since it's your follow up (1st C-ext is from date of application).

Most immigration offices allow you to apply 30 days before expiry date of your current stamp (you will not "loose" days).

yes I checked, I can not see your visa, just new stamp.

By the look of it you were first under consideration for a week and then someone decided to give you 60 Day covid extension

This is strange because usually the under consideration time is included or should Be included in your 60-plus days, now you got it even on top๐Ÿ˜ƒ

Normally you should have been giving an 30 day extension so consider yourself very lucky if this is straight on a TV.

When this is a 60day tourist visa (visa as sticker in his passport) then he is eligible for a normal 30 day (not Covid) extension. Just go to immigration

you're still not answering what KIND of visa, such as a tourist visa, special tourist visa, non-B visa, Non-O visa, visa exempt, etc. Although 60 days is most likely to likely a tourist visa but you still aren't answering the question people are asking.
