I just applied for an Elite Visa 2 days ago. There is an office a couple miniutes walk from the sky train you can apply right there.
They need a copy of your passport, and one passport photo in addition to the application.
While I was there 2 people brought in their passport, and said they needed to do a 90 day report. They take your passport and do it for you.

Yes. You can stay for up to 1 year without exiting, you need to report every 90 days when you do so. You do not need a new visa for 5 years. How is this inconsistent in the slightest? Though, it's worth noting 90 day reporting is very, very easy for Elite visa holders.
