What do I need to include in my OA Retirement Visa application for Thailand?

Hi there, I'm from Australia and am just doing my Visa Application for OA Retirement Multi Entry (forms from Sydney). I just have a couple of questions about the forms if anyone can help:

1. On the Visa Application do I need to enter a date of departure & Flight number under 'First Entry"

2. Do I need to put a Proposed Stay duration

3. On the Personal Data form, what do I put for proposed address to stay in Thailand

4. What do I put for Reference person in Thailand

5. Can the documents just be stapled together
