I believe in Bangkok you apply for the certificate, pay 200 baht and they post it to you within 7 days to the address on your TM30.

Not 100% sure that this is still the case.

The place where you stay should provide you one . I used one to get my Thailand license and another one for my extension. Both were provided from my hotel.
