that is the eEXTENSION service offered by VFS global which charges a FEE depending on how fast you want your extension approved.

The cheapest price point is 500 baht PLUS the 1900baht for the extension). You apply, upload the documents, get the approval then you go to the eExtension office (which is NOT at IT Square) but out at the government complex where the main immigration office is) on the floor above it

I'm confused as to whether Stamping Location refers to where I received my initial stamp (the airport) or where I'd like to receive my extension.

, if you are using eExtensioin, please contact to VFS directly. This group doesn't support applications for commercial services (i.e, eExtension) even under the Thai government.

I'm not sure what you're asking and it doesn't matter. As long as your most recent TM30 filed for you matches the province of the immigration office you go to apply for your extension, you should be able to get it there. Also Chiang Mai let's you apply when you have 45 days or less left.

does it matter how late I leave the appointment provided by this service? E.g. does leaving it closer to my stamp end date potentially cause me any problems?

As I'm planning to visit Chiang Mai next week for maybe 7 days. Would it be okay to book the appointment for when I return? Or is it best to do it sooner? (I've only used 3 days out of 30 so far)

doesn't matter when you do the extension, it's added to the expiration date of your current entry stamp

why not just get the extension in Chiang Mai? You can use the TM30 from the place you're staying up there and go to one of two immigration offices the main one by the airport or the one in Central Festival Mall

it's in a mall so if you go there before the mall opens, you can get in the door on the front of the building (the side that faces Chaengwattana Road) on the ground floor (actually you go in the loading dock door) to enter by the 7 Eleven.

Then you can walk to the left, take the lift up to the 3 floor (they have a label on the lift buttons so you know where you go (Counter K) and that will get you up there to queue up before the regular mall doors open

Last time I was there before opening time the buildings aircon was switched off. Be ready for a sweaty wait.

yes, first thing a little before they open even, and not on a Monday or Friday (or any day before or after they are closed if there's a holiday)
