I have a question to ask, I am Italian, can I apply for a multi-entry tourist visa online if I am outside Italy?

No. You cannot.

One of the requirements of a Multiple Entry Tourist Visa is that you can only apply through the Thai embassy where you are a citizen or have permanent residence.

A requirement of the e-visa system is you must physically be in the area served by the embassy you are applying at.

That means you can only apply through the Italian embassy for an METV and you must physically be in Italy when you apply.
