Why was I only given a 3-month Non-O Visa instead of the 12-month I applied for?

Jomtien immigration have only given me a non-O Visa for three months, I applied for a 12 month, has this happened to anyone before? 
And do I have to complete the forms and get bank statements again for when I return in 3 months time.

There is no 12 month VISA inside thailand 🙁 I think you don't understand how the process works
You FIRST apply for a 90 day Non-O visa for 2000baht, that goes under review for 2 weeks, then you go back to get the Non-O visa and a new 90 day stamp inked in
THEN you wait
until you have 30 days or less left on that 90 day stamp, get all new documentation, bank letter, TM30, lease, updated bank book, TM7 application for extension form, etc and go back to apply for the year "extension of stay" for 1900baht on the 90 days you got
They will take your application and you pick your passport with the extension in it up the following day
After that IF you need a re-entry permit to get in / out of the country the coming year you buy one (single re-entry 1000baht, multiple re-entry 3800baht)

no, you cannot, the 90 day Non-O visa application doesn't get an under consideration stamp 🙁 you apply and it goes under review for 2 weeks and then you go back to get the 90 day Non-O inked inyou have to stay IN the country until that visa is approved or you lose the application

you're right where you need to be
just burn down 2 months on that visa,
keep the funds in the bank too as it has to be in for 2 months and then get all new documentation
and
go apply for the year extension for 1900baht
remember though, if you need to leave the country during this 90 day stamp you're on you need to buy a re-entry permit or you cancel the stamp when you leave and will have to start all over again.

I was going to apply for my extension of stay on 4 April.I leave Thailand on 27 April, once I’ve got my 12 month retirement visa and my re entry permit

AGAIN: there is nothing such as a "12 month retirement visa" . . . . . . there only is a "12-months extended stay permit based on retirement" . .it is a stay permit, not a visa, the visa will become invalid after the "Extension" has been issuedTod *********

you can apply for your yearly extension any time you have 30 days or less left on that 90 day stamp (that expires May 4th) so April 4th will work fine Just realize that is a Friday and you won't be able to get the passport back until the following Monday
You might just hold off and go Monday the 7th, then you get the passport back from immigrations on the 8th

EVERYTHING done again, TM7 application for extension, the TM30, lease, copies, bank book, bank letter, etc and 1900baht the form is the application for extension form TM7 not the one you used this last time to get the in country visa
