Why do I need a QR code to fill out the Thai pass form, and is there a workaround if I don't have one?

Have been trying to  fill Thai pass form  .it keeps asking for  qr code which I don't have.  Is the request as per normal  ? . Is there a way around this.

. It's the vaccine certificate OR sometimes y'r insurance certificate. They also have a QR-code. (mine at least)
