Do I need a QR code for proof of vaccination when I apply for the Thai Pass? In the US we are only given a paper card.

I got my vaccine at walmart back in April. The guy who gave me the shots said the only record would be the CDC card. Well James Miller mentioned something that prompted me to login to walmart. I could get a printed version of my vaccine shots with a QR code. Also, I could export it into a mobile app called CommonHealth (probably others if there is a common format for the file type).
