so if my arrival time is 12:35 and I book the transport through my hotel, will they get there at that time or will they wait around for me to do PCR test and such? My concern is they'll get there and I won't be ready due to the tests and paperwork

The test at the airport is pretty straightforward and quick to perform. No hassle. And honestly, since you will be booked in a SHA+ hotel, you may as well book transport through them. Takes the onus off of you.

Hotel you are guaranteed if driver breaks down or faĺls ill, they wiĺl find a replacement.

When i came in with sandbox, hotel arranged driver was 1000 baht, cheapest private driver to Rawai was 850 baht.

6 to 700 baht i am not sure. They did make maximum prices when sandbox started to avoid scalping
