TLDR : Answer Summary
The TM form number for applying for proof of residency in Thailand is not standardized and varies by local immigration office. Some users report that Phuket and Hua Hin have their own specific versions of the form, and it is advised to check with the local office. Common requirements include passport copies, photos, and landlord documents.

From my experience, this process isn’t standardized. You can find the form’s name on the main immigration website, but the actual form, process, and document issued can differ depending on your local immigration office.
Surprise! You’ve just spent 200-500 baht to have your address written out in Thai. Absolute bullshit, I think a utility should suffice.

Hua Hin immigration requires copie of your landlords tm47, 2 passport photos and a copy of your passport bio and stamp pages to obtain a residency certificate for driver's license. Pranburi DLT will also want passport copies, and copies of f driving license from your home country. If you have an IDP, bring it and copies too.
for residency they didn’t ask for it,90 day report didn’t ask for anything other than form and passport,it was my second 90 day so maybe looked up on computer?
been Khon Kaen immigration today for residency letter, just one form name/address and passport,at the same time did 90day report same thing TM47 and passport .
I get so confused on which tm's. I thought a tm47 was the initial document and tm30 is what is filled for subsequent returns to an address... From perspective of leasee... My landlord gave me copy of tm47 and that's what I used for residency certificates, and iv'e not yet seen a tm30 document. But I also haven't stayed anywhere else yet.

It takes a couple of minutes to complete the application form, just your details and tick boxes

To my knowledge there isn’t one. Each office probably has their own form. There is one you can get online if you search but as an example Phuket won’t use that and you need to fill out their own form.
Your office may be different.
