TLDR : Answer Summary
The 'Form A' referenced at the Royal Thai Embassy in Canberra is a 'Personal Data Form'. It is not readily found on the embassy's website. However, one commenter provided a helpful link to the Retirement visa section where all relevant forms, including the 'Personal Data Form', can be found.
