Do yourself a Big Favor...get a NEW passport, then apply for the DTV visa.

Get another passport if you don't have much time or many pages left, otherwise you'll need to carry two passports for the next 5 years.

The application system tells you at the very beginning of the form (where you have to upload the biodata page of your passport), I think it was 6 months
