There’s an app called CamScanner. You can take a picture using your phone and it will convert it into like a scanned version. Then it can be put into a single PDF

Anonymous participant it needs to be one document which is best done with a PDF. Seems like this might be beyond your ability so go to a copy shop or similar and ask them to do it
