Not needed, but they will charge you an extra 200 baht for taking your photo and filling out your application. So 1200 in total.
multi not a good deal if someone doesn't make many trips or only has a short time left on the extension. One good thing with multi is that your passport doesn't get filled up.
Re-entry permits do not have a 1900 baht fee. Extensions cost 1900 baht. Re-entry permits cost 1000 baht for a single re-entry permit, or 1200 baht at the airport.
