What information should I provide on the travel information page when applying for the workation visa in Thailand?

Hey guys, I'm trying to apply for the workation visa and got stuck on this page.

What did you all enter on the travel information page?

1) From what I've seen people saying, there is no proof for flight tickets necessary. So I just look up flights and provide airline and flight number? What did you insert at "date of departure" if you intend to extend for another 180d within the country.

2) "Have you ever applied for Thai visa"

Does this include previous tourists visas or only long term visas?

3) Hotel accommodation

Do I need to book a hotel and provide the booking confirmation? The field even asks for a phone number so I'm not entirely sure if I should just input the hotels phone number here.

I couldn't find anything specific in the search so I'm asking these questions in case others may also have issues here.
