What information should I provide for Visa on Arrival application in Thailand?

Help !!!

Guys need information regarding Filling up Visa on arrival application.

What to write in 

1. Address in Thailand

2. Name and address of person reference in Thailand.

I guess for question number I have to write the Hotel/Hostel address. But m confused about question number 2. Please help me in this regard.

And yes one more query

We traveling in a group of 5 so to obtain VOA do we need 5copies of documents for 5 person or single copy will be fine for all?

TLDR : Answer Summary

When filling out the Visa on Arrival application, for the address in Thailand, write the hotel or hostel address where you will be staying. For the second question regarding the name and address of a person in Thailand, include the name and address of a contact or the person you plan to visit if applicable. Each individual in your group needs to provide separate copies of the required documents, as the application must be submitted in person for each person.

Requirements for a Visa on Arrival:

Show of proof of hotel accommodations for 15 days or letter of invitation, name and address must be filled into the highlighted part.

Show of proof of funds, every person needs to show a minimum of 10.000 Thb or equivalent currency in cash.

Proof of onward travel within 15 days.

This is a VISA application and every person ,who needs this VISA must apply in person.

Visa is for 15 days and can not be extended at the Thai Immigration Office.

With 1 application you get 1 Visa on Arrival and 1 person can enter Thailand.

If 2 persons fill in this application and show the proofs, 2 persons can get a Visa on Arrival and enter Thailand.

If 3 persons, I guess you start to understand.

There is no group visa on arrival method. Each for themselves.

Every person applies for his own Visa on Arrival. Be aware that also every person needs to bring a passport picture, group pictures or mobile phones with a camera does not do the trick.

Let us start with what is the country of origin of your and your friends passport?

The document you show is for application of a Visa on Arrival, which in for only 21 counties available. To look up for yourself, your country must be mentioned in the first column colored light green

I think it refers to where you are staying in Thailand. And name and address of person you are meeting in Thailand .
