For Jomtien Immigration office, same requirements as if you are applying for a 30 day extension of stay. But your application is DENIED...And you are ordered to LEAVE Thailand WITHIN 7 days off the date you apply for the extension.
So time your application carefully. Make sure you have a TM 30 registration of your address on file with Thai Immigration and show PROOF of your flight departing Thailand within the 7 days.

Not sure about specifics in Jomtien but in Phuket along with the regular stuff needed for an extension you will need to fill out the form STM2 and provide a copy of your ticket out of Thailand.

it's not a 7 day extension it's an "application for extension denied" stamp that gives you a week to get out of thailand,
you supply the same copies and TM7 that you do for the regular 30 day extension. It's 1900baht just like all extensions are
