What documents do I need to obtain a DTV visa through a Muay Thai gym in Phuket?

For those who went through the Muay Thai path.

-Did you get your classes from a gym licensed with the Sports Authority of Thailand? (The gyms near where I am trying to train in Phuket don't seem to know anything about this)

-Was proof of buying the classes enough? Or did you also have to get other documents from the gym (Such as owner info, certification license, custom letter of acceptance, etc)

Ali's Boxing Gym helped me get mine, they knew all the paperwork I needed and my application was smooth.

Immigration emailed me requesting more documents and I was able to obtain this from Ali’s Boxing and I got approved the next day

I've also been looking in Phuket and there are options in Rawai, Karon, Kathu and Patong. They're sprinkled in various comments so I think first it's important to know where you're located as you probably want to go with the place that is closest to you

I've been looking in Bang Tao, I have asked 3 gyms already about being certified, but only 1 has responded that they don't do Visa (no word on being certified), the other 2 have yet to respond

the nearest one to you would probably be Patong. I would rather go with a gym that knows what they're doing and has experience handling the visa to not complicate things.
