Hi, from Muay Thai Fever Gym in Chiang Mai. We have successfully provided documents for over 30 DTV visas. We get asked if we have branches outside of Chiang Mai everyday. Sorry we don't, but if you want to find a gym in Bangkok, Pattaya or Phuket I would advise you to make sure they have the following docs. 1. Sports Autority registered gym. 2.Coach is Board of Boxing Sport A, B or c license (pref A license). 3. Gym is a Ltd company registered for tax. 4. Ministry of Education Skills Development certificate. If you can find a gym with all those documents and they package it correctly you will have no problems. Its not to say a gym is no good if it does not have this level of compliance and it doesn't mean you wont get accepted without these docs but if the gym gives you all those you cant go wrong really. Saves the ping pong backward and forward between gym and embassy. Also get a 6 month course, again you might be ok without it but if you have a 6 months course you wont get a comeback. Hope this helps.

TLDR : Answer Summary
The Muay Thai Fever Gym in Chiang Mai provides over 30 DTV visas and advises potential applicants on what to look for in gyms outside Chiang Mai. Essential requirements include a Sports Authority registered gym, a coach with appropriate Board of Boxing licenses (preferably Level A), a gym registered as a Ltd company for tax purposes, and a Ministry of Education Skills Development certificate. Having a 6-month course can further enhance your chances of recognition, though it is not always strictly necessary.
