What documents are required to submit a TM30 application by mail in Thailand?

TM30 by mail question

First, tnx to Tod Daniels for confirming this this possible. Now, I prepare documents to send (Chaeng Wathana), please check if something missed / not required

1. Chanote copy (I owner)

2. Yellow Book copy

3. Passport copy

4. Departure card copy

5. TM30 filled

6. Return envelope with address and 10 THB stamps attached

Send this to address mentioned before fo TM30, and expect in return slip
