TLDR : Answer Summary
To file the TM47 form in person at the immigration office, you typically need the following documents: 1) Completed TM47 form, 2) Copy of your visa, 3) Copy of the passport page with your personal information, 4) Copy of the entry stamp in your passport, and 5) TM30 form for your last address. However, specific requirements may vary by location; for example, Jomtien has additional requirements, while in Chiang Mai, only the TM47 form and passport may be necessary.

It depends which office. They often have different requirements. Jomtien requirements in the pic. In Chiang Mai all you need is passport and TM47 which you can get at the office.
