What do I need to testify on the Testimony Memorandum for a police clearance check in Thailand?

I have a question regarding the Kakaesque nightmare that is applying for a police clearance check in Thailand from abroad.

I've got two sets of fingerprints made, and filled out the forms except the Testimony Memorandum where it says 'I testify that .... '

Anyone have any clue what it is I'm supposed to testify? That I haven't committed any crimes? That the fingerprints I've had made are mine? I can find no guidance anywhere about this. Any help would be greatly appreciated!

I'm baffled by this I asked about it a few years ago the reply I got was could you enter Thailand obviously I could so why not able after applying I still not received positive answer
