TLDR : Answer Summary
If you hold a retirement O visa with multiple entries, you generally need your passport and the re-entry stamp. However, if your visa originated from an OA visa, you will also need health insurance. Additionally, be prepared to present a vaccination certificate showing at least two shots.

If you originally had an OA visa they you got from the US and then traveled to Thailand and extended that OA for a year and your non-O extension is based originally on that OA, then you are required to have insurance.
If you have never had an OA visa, then you just need your passport with your re-entry stamp.
