Two questions:

1. Should I reserve a "family" appointment if attending with my Thai wife? Or is "single" ok? (not quite sure what their warning in yellow means)

2. No appointment openings in all of November. Is this their way of saying you just have to come as a walk-in?

I’ve always booked single when I was doing the extension based on Thai family, my wife. Never a problem.

Your wife has to go with you regardless. You cannot apply for an extension based on a person without that person being with you.

Appointments open at midnight each day a month in advance. If you want any chance of getting one, you'll want to be waiting at 12:01am so you can grab the new day when it's made available. Just make sure it's a weekday or you'll be waiting for nothing. They fill up fast.
