Not sure for 30 day extension. Always did year extensions.The picture of you is one side of the STM2 form.
Other side is Acknowledgement for permanent stay so think that's not needed for sure. For Korat we also have a deposition form which you also not need.
Think the TM7 both sides is okay and maybe the overstay part which you showed.
