I have found it takes on average of 35-50 mins, depending on how many people are ahead of you. Pro tip: print the form online + bring your own picture to save 100-200 bht (single vs multi permit).

I had to wait about 30 minutes one time, but unlike David I was sent to the front of the immigration queue so total was about 35 minutes. I'd be cautious and take his advice though, they may send you to the back of the queue after getting your reentry stamped in

It's same hall as immigration. Got it in 10mins but after getting it had to go back in normal immigration line. So I'd take an extra 30mins if I were you.
