How can I obtain a re-entry stamp at Chiang Mai Airport for my Non-Imm O visa while flying out of Thailand?

I'm presently in Chiang Mai on a 90-day, non-imm O visa. I plan to fly out of Thailand for a few days next week. I need to get a re-entry stamp before I fly. As I recall, the immigration desk to do this, at CM Airport, is just past international security on the left. Is the desk still there and what forms do I need to get the re-entry stamp?
