How can I obtain a re-entry permit on a non-ED visa in Thailand?

Happy Songkran everyone! I want to get a re-entry permit on a non-ED visa to apply for a third-country visa inside Thailand. 

Shall I go to the IMM office where the non-ED visa was issued? Or I may get it from IMM anywhere? 

And what shall I do when I apply for the re-entry permit in the IMM office? Is it easy to get it by myself without any condition? 

Can I also do it for my wife without the need of being presented by herself?

From this photo, in the content of the request documents, there is an extension stamps page. Does it mean I need to copy all the stamps since my latest entry, including the entry stamp, following extensions, and current non-ED?

could be like that. Or maybe only the latest extension. Please check with your local IO.

As a rule you go to the immigration office where the extension was issued to get a re-entry permit. Some offices will do it even if your extension wasn't issued there BUT some won't

You have to apply in person so if you're getting a re-entry permit for your wife she goes to

OR

You both just get re-entry permits when you are at Suvarnabhumi airport on the day you fly out. Re-entry permits are sold
