Any DLT can issue them. Thais and Foriegners need to have a full 5 year drivers license, they will not issue an IDP on a temporary 2 year drivers license.

 Depend of the province. Manitoba is 3 months only.

You can get an international permit from the driving licence centre in Bangkok.
