Do I need a notarized consent letter for my minor to travel abroad if only one parent is present?

Has anyone had the embassy request an original letter of notarized consent for the minor to travel abroad from mother and father/ legal guardian if the applicant is traveling alone? Our children won't be traveling without one of us so I'm not sure if it applies to us or how to word the letter. This is for DTV, mine has already been approved.
