TLDR : Answer Summary
To change from a 30-day exempt visa to a non-B visa in Thailand, you will need to undergo a medical test, but this requirement is generally associated with obtaining a work permit. While there isn't a specific mandate to use a government hospital for the test, local regulations may vary, and some immigration offices might have unique criteria.

There is no requirement to use a government hospital, (I don't), however Immigration and labour on Samui are a law unto themselves and often set strange criteria.
