This statement is required by immigration at ChangWattana for a marriage visa. My question is can my wife be the guarantor or does it have to be a third party? This form was given to me when I went to CW to make enquirers regarding the documents required.

you fill it out yourself, it's required for visa extensions based on marriage, retirement and has been for a good number of years now. Most people get it handed to them when they're sitting at the officers desk and she says write your name on it and sign it
