Looking to apply for a DTV with using my an appointment slip from BKK hospital for my annual physical. Anyone see a problem with that?

I have obtained with hospital appointment slip and visa invitation letter.

To play it 100% safe, you will need to bring the following:

-Letter of doctor stating the treatment and appointment(s)

-Receipt

-Appointment card (could be mentioned in the letter

- Medical certificate

All stamped with the hospital or clinics official stamp.

Depends on the embassy, some will and some will not, Ask the embassy you will apply from for what they need to give approval.

I think it is a bit of a stretch asking for a 5 year multiple entry visa for a single annual medical appointment but give it a go and let us know how it works out :-)
