Looking to apply for a DTV with using my an appointment slip from BKK hospital for my annual physical. Anyone see a problem with that?

To play it 100% safe, you will need to bring the following:

-Letter of doctor stating the treatment and appointment(s)

-Receipt

-Appointment card (could be mentioned in the letter

- Medical certificate

All stamped with the hospital or clinics official stamp.

I think it is a bit of a stretch asking for a 5 year multiple entry visa for a single annual medical appointment but give it a go and let us know how it works out :-)
