I have obtained with hospital appointment slip and visa invitation letter.

My application failed in Vientiane because I only produced an appointment for surgery letter and not documents about follow up appointments.

-Letter of doctor stating the treatment and appointment(s)

-Receipt

-Appointment card (could be mentioned in the letter

- Medical certificate

All stamped with the hospital or clinics official stamp.

I did apply with appointment slip from Bangkok Hospital for a dentist، and they accept it and i got my visa.

I think it is a bit of a stretch asking for a 5 year multiple entry visa for a single annual medical appointment but give it a go and let us know how it works out :-)
