TLDR : Answer Summary
Yes, you can convert your German driver's license to a Thai one in any province, not just Bangkok, provided you have all the required paperwork. You will need documents such as a Certificate of Residence, medical certificate, and your original driver's license (in English or translated). It's also noted that some provinces might have different requirements or fees, and it may be quicker to use an agent to avoid long queues.
I got a car and bike licence on a tourist visa for 2yrs. Just renewed, for 2yrs on a tourist visa. U need retirement visa or equivalent to renew for 5yrs. Use an agent pay the money alot less hassle
Be advised that since 16 July, you have to be on 12 month extension of permit of stay based on Non-Immigrant O visa in order to get the 5 year extension. In case you are still on initial 90 days permit of stay, only 2 years extension will be granted. This is according to my own experience last week.
It may be of some help, at least some orientation about converting driving license (that translation mentioned below is done by the agency, no need to do it via embassy yourself):
Here’s the process:
1. Book an Appointment: You’ll need to schedule a visit with us. The process takes about 2 hours, and my staff will meet you directly at the DLT.
2. No Exam Required: There’s no driving test needed, just a physical test for your eyes and reflexes.
Required documents (no copies allowed):
- Passport & Visa
- Original driver’s license from your home country (must be in English or have an official translation certified by your embassy)
- Proof of residence (a certificate from either Thai Immigration or your embassy)
- Medical certificate (we can arrange this near the DLT on the same day)

Anyone knows if the “certificate of residence” can be replaced by a workpermit? As your workpermit is linked to your adress already.
It's an embarrassment that to book an appointment in cities takes 3-4 months which I did in Phuket to renew, other option is to pay agent, done within a few days

Based on recent experience - you will need to show your host Country licence (usually in English) and normally an IDP linked to it, Passport, Visa Certificate, Certificate of Residence (show to immigration your Passport, Rental Agreement if Renting, 2 x Passport Photos, Visa Certificate) - if you wish for a car and motorbike licence, you will need a Certificate of Residence for each at 500THB each), Medical Certificate (no longer than 30 days old), and complete the one hour course online (remember to download and take the receipt with you in your smart phone) - the cost is in obtaining the documents, each licence (Car & Motorbike) are 10THB each.

I should add: once I satisfied the document checks as outlined, I then had to fulfill some practical reaction tests. Altogether, I was in and out with both Thai licences within 2 hours.
last time I got a residence certificate in Sa Kaeo it was 500 baht but that was around 5 years ago. The DLT are great but they do want people to be on Non-immigrant visas
understand, I did not need to sit any written tests etc. I used my Malaysia licence with IDP to acquire a car and motorbike licence in Thailand. 10THB for each licence - the cost was in obtaining a medical certificate, certificates of residence etc.
You can do in any province if you got all documents they want. Or use the agent they might have queue for you . And yes you do yourself is cheaper.

My Thai wife didn’t make an appointment to get her driver’s license. I won’t make an appointment when I get my driver’s license. We are in Ratchaburi province.

You can get a license from any DLT office. Just make sure you have all the paperwork (Certificate of residence, health check, any other items the DLT you will go to requires).
The driving license has to be translated in Thai. With that and other documents, I was told by an agency they can make my Japanese driving license converted to Thai DL within 2 hours (they do the translation) with their staff going with me, eye sight and reflexes test (a doctor from the DL Authority checks for that).
The price is 4,400 THB, my American neighbour told me it is a fair price. No theory nor driving test.
